Closure Is a Decision, Not Just a Technique
Wound closure is often taught as a single skill, but in practice it's a series of decisions: how the wound should heal, what layers need to be addressed, what material and pattern fit the tissue, and how much tension the closure can safely tolerate. Understanding the reasoning behind those decisions — not just memorizing a stitch pattern — is what separates a technically correct closure from a durable, well-healed one.
The Three Types of Wound Healing
- Primary intention. Wound edges are approximated and closed directly — the standard approach for clean, sharp lacerations and surgical incisions with minimal tissue loss.
- Secondary intention. The wound is left open and heals by granulation and epithelialization. This is typically used for contaminated or infected wounds, or where there's too much tissue loss to approximate edges safely.
- Tertiary intention (delayed primary closure). The wound is deliberately left open initially — often to monitor for infection or allow swelling to resolve — and closed surgically a few days later.
Choosing the right approach starts with an honest assessment of the wound: contamination level, tissue viability, time since injury, and blood supply all factor in before a needle is ever threaded.
Layered Closure: Working from the Inside Out
Deeper wounds are rarely closed in a single layer. A typical layered approach addresses:
- Deep/fascial layer — carries the bulk of the wound's tension and is closed with stronger, often absorbable suture.
- Subcutaneous layer — closes dead space to reduce the risk of seroma or hematoma formation.
- Skin/epidermal layer — the visible closure, prioritizing edge approximation and cosmetic outcome.
Closing dead space matters more than many new learners expect — fluid collecting in an unclosed space beneath the skin is a common driver of wound breakdown and infection, even when the visible skin closure looks technically excellent.
Tension Is the Enemy of a Good Scar
Every closure balances two competing goals: enough tension to keep the wound edges together, and little enough tension that blood supply to the wound margin isn't compromised. High-tension closures are more likely to result in wound dehiscence, spreading scars, and tissue necrosis at the edges. This is why deeper-layer closure matters — distributing tension across layers, rather than asking the skin closure alone to hold the wound together, produces a more reliable result.
Signs a Closure Needs Reassessment
Part of wound closure competency is recognizing when the standard approach isn't appropriate — heavy contamination, questionable tissue viability, signs of active infection, or a wound presenting well outside the golden period for primary closure are all situations that call for a different plan, not a routine closure.
